Erin

Award Category - Exceptional

Erin, Azura of Eau Claire: Erin is recognized for providing exceptional support to all of our residents and helping them to RECOVER during difficult times. One day, a resident had asked the team how his friend was doing as he had not been at the table for meals. The team explained to him that he was not doing well and after hearing this, the resident asked if he could go see his friend. Erin got the OK and brought him down to his friend’s room herself after supper. He was able to sit by his friend’s bedside, say the rosary and a prayer and say his goodbyes. A team member passing by witnessed this moment and joined in in talking about all the good times and laughs they had had with him and how much they were going to miss him. This resident passed away the following morning.  Thank you Erin for supporting this resident and helping him to have this special moment with his friend. You are exceptional!

Trevor

Award Category - Exceptional

Trevor, Azura of Clinton: Trevor is recognized for his EXCEPTIONAL efforts in providing support, compassion and helping our residents to RECOVER. One day, the team noticed one of the residents had been standing by the door and wanting to leave, telling the team that she was waiting for her party of 8 to pick her up. Trevor heard this and told this resident he would, with the lights and decorations and poinsettia flowers, be more than happy to accommodate her and her party. She agreed and with that, Trevor got out card tables, 8 chairs and laid 8 full place settings; even though it was just this resident and two other residents that had joined. After, this resident told Trevor that she had a wonderful evening and said that her whole party really enjoyed themselves. Thank you Trevor for being exceptional and putting in the extra effort to make a true MOSAIC moment happen for this resident.

Valeria

Award Category - Retain

Valeria, Azura of Oak Creek: Valeria is recognized for always smiling, making sure the residents look good, and for never hesitating to step-in when needed. She shows respect to all and has taken the time to make each new resident feel right at home. When a newer resident had stated that she wanted to go home, Valeria took the time to sit and have a conversation with her about her and the Mexican culture. It turned out that this resident has family in the same city her family lives in in Mexico. Valeria reassured her that the people at Azura were friendly and going to treat her with respect. This personal connection helped the resident feel more comfortable and at home in her environment and two days after the conversation, the resident had not stated that she wanted to leave. Thank  you Valeria for assisting in the RECOVERy of those within our care, we RETAIN the finest in you!

Shamika

Award Category - Retain

Shamika, Azura of Oak Creek: Shamika is recognized for being an amazing team player, covering shifts when needed and always having a positive attitude. She was new to caregiving when she joined the team in October, but her team members share that she has done amazing  and when she comes on shift, is willing to help out as much as possible. She is always there to answer questions anyone has and is always making the residents laugh, and according to one nomination, “most definitely transforms the culture of memory care.”

Rhonda

Award Category - Innovation

Rhonda, Azura of Manitowoc: Rhonda is recognized for showing INNOVATION in trying to come up with new ways to help our residents. She helps them in making holiday decorations and has researched and proposed to buy more baby dolls for the home after discovering that they bring joy to  many of our residents. Rhonda has also taken the initiative to assist in retaining the finest people.  Rhonda has given new team members rides to work as well as mentored them and made herself available when they have had questions. Thank you Rhonda for all you do in providing an exceptional environment for our residents and your team members!

Landon

Award Category - Innovation

Landon, Azura Support Center: Landon has shown INNOVATION in working to improve the printing of the Azura training manuals. He worked with the appropriate parties to first see if it would be acceptable to do an electronic version to keep instead of a printed one and then searched to find the most cost effective, reusable manuals for classes and worked to try and get them created for each Trainer. Thank you Landon for using INNVOATION to help improve Azura’s training program!

Latonya

Award Category - Recover

Latonya, Azura of Kenosha: Latonya is recognized for her willingness to assist and aid in the recovery of her fellow team members in dealing with the loss of a resident. Latonya had come to the home at 2:30 AM in order to help support the team and made sure all knew what to do when this difficult time approaches. Thank you, Latonya, for being there for our residents and for showing care, compassion and support in helping your fellow team members recover during these tough and emotional moments.

Leslie

Award Category - Recover

Leslie, Azura of Manitowoc: Leslie is truly amazing when it comes to RECOVERy.  She not only assists in the recovery of our residents but also helps our Azura family members in making sure they feel right at home. She always engages in conversation and makes sure they have coffee or a snack when they visit.  Recently, she helped ease the transition of a new resident moving in by taking the time to get to know her and incorporating her in the daily operations of the house after discovering her love of assisting with laundry, dishes and meal preparation. Leslie consistently praises this resident for her assistance which has helped her feel more at home. Thank you Leslie for ALL you do to help our residents and families RECOVER!

Jeremy

Award Category - Recover

Jeremy, Azura of Beloit/Clinton: Jeremy is recognized for helping an Azura of Clinton resident during his transition to a new bedroom. Jeremy took the resident to the new room and showed him all the “cool” things about it and kept him engaged. Jeremy even got him a new shelf for his belongings and made sure to show him his shelf, which he loved. The team followed in Jeremy’s footsteps and kept the transition positive and even got him balloons to celebrate. Thank you Jeremy for all you did to aid in the RECOVERy of this resident during this transition; it shows how exceptional you are!

Angela, Azura of Oak Creek: Angela is recognized for all of the ways she demonstrates RESPECT to her residents, our Azura family members and her team. She takes the time to personally welcome new residents into our family of care, making sure to sit and chat with them and engage with our residents to bring them joy. Angela is compassionate, both in the care she provides and also in her training of new team members. Thank you Angela for embodying the Azura way and showing RESPECT to all!
